若導師及任課老師觀察到學生在學習適應上有顯著困難(請參考附件圖檔)

請協助收集《提出轉介前需準備資料》所列出各項資料

並請在提出轉介前填寫

1.特殊需求學生轉介表-100R

2.疑似生初篩轉介表

檢附相關資料後送至四樓輔導室特教組

會再跟老師約時間了解學生狀況及確認學生學習資料

特別重要的一點是

老師們在提出轉介前請先詢問"學生"及"家長"意願

並清楚說明轉介目的

附上相關附件，若有疑似鑑定問題在請撥至分機407
