花蓮縣110年度國民中學學術性向資賦優異（數理類）學生鑑定安置初選評量學生報到與應考注意事項

報到時間：110年03月06日（六）8：30～9：10。

(入場證上的學生報到時間原訂8：50～9：10，但此次評量由於需量測體溫，而且如有發燒情形另需填寫健康控管單，因此建議家長務必提早至8：30進行報到，以利評量進行)。

報到地點：自強國中一樓穿堂。
學生報到時應完成下列工作：

(1)配戴口罩

(2)量測體溫

(3)繳交「實名制健康關懷問卷」(請家長先下載並於家中填寫，並於報到時繳交，如未繳交需現場填寫)。

(4)於考生簽到表中簽到。

(5)出示入場證，以利查驗身分(未攜帶入場證不得應試)。如果入場證遺失，應準備與報名表相同的大頭照辦理補發。

(4)如果學生有發燒現象 (額溫≧37.5度/耳溫≧38度，並且複量確認後確實有發燒現象)，須於現場填寫「健康控管單」，仍有鑑定意願者應接受試務中心的安排至備用試場(801教室)應試，並全程配戴口罩，且休息時間不能與其他人交談，以降低傳染風險。

因應疫情，此次評量「不提供家長休息室」，請家長確認學生報到成功後即可離開，不要進入校園環境。
請學生自備評量用品，評量進行中不得向他人借用。
可使用透明墊板，但是墊板上不得有任何文字與標記。
請學生全程配戴口罩，本校不主動提供口罩。
請勿攜帶電子裝置入場(如：手機、i-watch、小米手環、小米手錶…)。
中午不提供便當，也不提供代訂便當服務，中餐請自理。
